Q3 2026 Earnings Call Summary
The call was predominantly positive, with solid third-quarter results, improved clinical outcomes, strong expanded HD adoption and rollout readiness, significant IKC earnings growth, continued international improvement and maintained full-year free cash flow expectations. These positives were tempered by higher technology spending, below-historical treatment volume growth, missed-treatment pressure, commercial-mix and ACA headwinds, higher operating costs and uncertainty around future ESRD reimbursement adjustments and policy economics.
For full year 2026, DaVita now expects adjusted operating income within a range of $2.175 billion to $2.25 billion, adjusted earnings per share within a range of $14.30 to $15.20, free cash flow between $1 billion and $1.25 billion, IKC adjusted operating income of at least $65 million, and approximately $135 million of international adjusted operating income; the company expects a $40 million year-over-year headwind from ACA exchanges, an approximately 1% headwind to fourth-quarter treatment volume from year-over-year day mix, and treatments per normalized day up more than 1% in Q4, while 2027 volume growth is expected to be higher than volume growth in 2026, pharmaceutical costs are expected to come down, and IT spend is expected to decelerate from 20-plus percent growth but continue to grow. Expanded HD is expected to have no material net impact on adjusted operating income for 2026 or 2027, with the positive impact of lower mortality and higher volume growth beginning in the back half of 2028 and supporting a path back to 2% or greater volume growth by 2029. For full year '27, the estimated net impact of the ESRD final rule is 1.5% to 2%, with 1% worth about $30 million a year, while the 40 basis points excluding TDAPA is fixed for the year and the 3.1% TDAPA component applies only to Q1 and is expected to come down significantly in Q2 and stay down and potentially further decline over the course of the year.
- Improved Fluid Management and Patient OutcomesSpecialized clinical protocols, teammate education and proprietary digital tools produced a 10% improvement in treatments achieving optimal fluid removal, a 5% improvement in patients' management of weight gain between treatments and nearly a 5-pound reduction in average patient weight. Management said these advancements are supporting continued mortality improvement and better daily quality of life.
- Expanded HD Dialyzer Pilot Shows Strong AdoptionDaVita completed expanded HD dialyzer pilots in 150 centers involving more than 10,000 patients. Approximately 75% of total patient census is clinically eligible, and adoption among clinically eligible patients in the pilot exceeded 90%. Patients reported feeling better and having more energy compared with prior therapy, while patient and physician feedback was described as overwhelmingly positive.
- Expanded HD National Rollout Ready to BeginMore than 95% of clinical teammates nationally have been trained on expanded HD, and the existing dialysis machines are compatible with the new dialyzers. DaVita plans to begin the national rollout the following week and expects to bring expanded HD to the vast majority of eligible patients in November.
- Expanded HD Supports Long-Term Volume Growth ObjectiveManagement does not expect a material net impact on adjusted operating income in 2026 or 2027. It anticipates positive effects from lower mortality and higher volume growth beginning in the back half of 2028 and growing over time, representing a milestone in the path back to 2% or greater volume growth by 2029.
- Solid Third-Quarter Financial ResultsThird-quarter adjusted operating income was $561 million, adjusted earnings per share was $3.82 and free cash flow was $276 million.
- Integrated Kidney Care Outperforming PlanIntegrated Kidney Care was above plan in the third quarter and is on track to outperform its original full-year guidance. DaVita now expects at least $65 million of 2026 IKC adjusted operating income, an increase of at least $25 million over prior guidance and $45 million above 2025. The improvement was driven largely by a higher shared savings rate in the CKCC business and Medicare Advantage value-based care contracts.
- IKC Expansion and Clinical ProgressThe Humana partnership is expected to expand upstream care to approximately 10,000 chronic kidney disease patients. Management said IKC clinical interventions are reducing avoidable hospital admissions and readmissions while increasing planned optimal dialysis transitions, improving patient outcomes and generating shared savings for CMS and Medicare Advantage health plan partners.
- Technology Investments Delivering Clinical and Financial BenefitsDaVita has invested more than $1 billion over the past few years to modernize its technology platform. Management said the investments are contributing to improvements in mortality, revenue operations, labor productivity, IKC care management and pharmacy costs, and expects the growth in technology investments to decelerate going forward.
- International Operating Income IncreasedThird-quarter international adjusted operating income was $46 million, an increase of $20 million year-over-year, largely reflecting growth and improved performance in the Latin America businesses. Full-year international adjusted operating income remains expected to be approximately $135 million.
- Share Repurchases ContinuedDaVita repurchased approximately 600,000 shares during the third quarter and an additional 500,000 shares after quarter-end, including shares from Berkshire Hathaway. The company finished September with a leverage ratio of 3.38x consolidated EBITDA, approximately flat versus the prior quarter.
- Strong Full-Year Free Cash Flow OutlookDaVita narrowed its 2026 guidance to adjusted operating income of $2.175 billion to $2.25 billion and adjusted earnings per share of $14.30 to $15.20, while maintaining expected full-year free cash flow of $1 billion to $1.25 billion.
- Positive ESRD Final Rule DevelopmentsCMS permanently incorporated phosphate binders into the dialysis bundle. The headline rate increase for large dialysis organizations is 3.5%, up 1.5% from the proposed rule. Management's current estimate is that the net impact for DaVita in full-year 2027 will be 1.5% to 2%, which it described as more than 50 basis points better than the prior rule.
- Support for Kidney Care National Plan ObjectivesManagement said it shares the administration's goals of earlier intervention, slowing disease progression, improving the transplant process, expanding patient choice, fostering innovation, investing in early detection and prevention, increasing transplant access and reducing regulatory burden.
- Broad Rural Access and Long-Term Mortality ImprovementManagement stated that 92% of patients in rural communities are within 10 miles of DaVita centers and that mortality has declined by roughly 25% when measured from 2000.
- Charitable Premium Assistance Program Has Favorable Review HistoryManagement said investigations, inspections, rule-making and legislative scrutiny of the American Kidney Fund charitable premium assistance program concluded that the grants benefit a vulnerable population and that the program follows the rules. The disclosed program figures have remained completely flat for approximately eight years.
