Q3 2026 Earnings Call Summary
The call was broadly positive. Zimmer Biomet reported solid third-quarter growth, strong performance across Hips, S.E.T. and technology, significant cash generation, progress in its commercial transformation and multiple increases to its 2026 outlook. The main challenges were declining knee sales, China-related headwinds, margin pressure from manufacturing costs and ongoing investment in the U.S. sales force. Management expressed confidence in the long-term strategy and expects improved knee performance and additional growth catalysts in 2027.
Zimmer Biomet raised its 2026 organic constant currency sales growth guidance to a range of 3% to 3.5% from 2.25% to 3.25%, adjusted earnings per share guidance to $8.50 to $8.60 from $8.47 to $8.59, and reported sales growth guidance to 4.4% to 4.9% from 3.9% to 4.9%; foreign exchange is expected to be an approximate 25 basis point tailwind, Paragon 28 to contribute 110 basis points, China-related headwinds to total nearly 100 basis points in the fourth quarter and roughly 50 basis points for the full year 2026, adjusted gross margin to be around 72%, adjusted operating margin to decline a little more than 50 basis points year-over-year, net interest and other nonoperating expenses to be $300 million, the adjusted tax rate approximately 18%, share repurchases approximately $1 billion, fully diluted weighted average shares around 193 million, and free cash flow to grow 9% to 11%; pricing is expected to be flat to 100 basis points of pressure, the company aspires to a 6% WAMGR by the end of the decade, expects 2027 to be a better year compared to 2026, and anticipates Monogram can launch in the first half of 2027, pending approval.
- Solid Third-Quarter Revenue GrowthThird-quarter total net sales were $2.63 billion, representing 3.1% reported growth and 3.7% constant currency growth versus the third quarter of 2025. Excluding the negative impact of the China distributor network consolidation strategy, constant currency growth would have been 4.7%.
- Broad-Based Portfolio PerformanceTwo-thirds of the company's book of business grew at upper single-digit or double-digit rates. Hips and S.E.T. grew at upper single-digit rates, while technology and data, bone cement and surgical grew in the teens.
- Hip Business Growth and Product AdoptionGlobal Hips grew 7% on a constant currency basis, including 5.8% growth in the U.S. and 8.2% internationally. Z1 represented over 50% of all U.S. hip stems, up from about 40% in the second quarter, while HAMMR was used in nearly 30% of U.S. primary hip cases. OrthoGrid delivered its strongest quarter to date, with over two-thirds of sales coming from recurring revenue.
- Strong International Hip MomentumThe iodine-coated hip stem and cup launch in Japan exceeded expectations, with supply being increased to meet greater-than-expected demand from existing customers and new accounts. The company is also working on launch plans for other geographies outside Japan.
- S.E.T. Growth and Paragon 28 PerformanceS.E.T. accelerated to 7.1% constant currency growth, including mid-single-digit growth in the U.S. Key franchises including foot and ankle, sports medicine, CMFT, craniomaxillofacial, thoracic and upper extremities grew between mid-single digits and high teens. Paragon 28 sales grew over 18%, or approaching 20% in other comments, and were described as growing roughly three times the foot and ankle market.
- Technology and Data ExpansionTechnology and Data, Bone Cement and Surgical grew 13% on a constant currency basis. Robotics, data and AI solutions grew 20%, and technology and data sales increased over 30% in the U.S., led by demand for ROSA with OptimiZe and TMINI.
- Raised 2026 Organic Growth GuidanceThe company raised its 2026 organic constant currency sales growth guidance to 3% to 3.5% from the prior range of 2.25% to 3.25%. Organic constant currency growth was 3.5% year-to-date after three quarters.
- Raised Revenue and Adjusted EPS OutlookReported 2026 sales growth guidance increased to 4.4% to 4.9% from 3.9% to 4.9%. Adjusted EPS guidance increased to $8.50 to $8.60 from $8.47 to $8.59.
- Improved Gross Margin OutlookFull-year adjusted gross margin is now expected to be around 72%, which is 100 basis points higher than the prior expectation, driven by manufacturing efficiency initiatives and favorable product mix.
- Strong Cash Generation and Share RepurchasesThird-quarter operating cash flow was $560 million and free cash flow was $372 million, both up about 34% year over year. The company ended the quarter with approximately $400 million in cash and cash equivalents, repurchased $79 million of shares during the quarter after $500 million in the first half, and remains on track to complete approximately $1 billion of share repurchases in 2026.
- Free Cash Flow Growth OutlookThe company continues to anticipate full-year free cash flow growth of 9% to 11%. Management also reported free cash flow conversion above 100% in the quarter and 77% year-to-date.
- Progress on U.S. Sales Force TransformationThree quarters into an eight-quarter transition, over 75% of the U.S. sales force is fully dedicated. The transition is proceeding ahead of expectations, disruption has been less than initially anticipated, and sales representative turnover remains at healthy single-digit levels.
- Expansion of Growth-Oriented Commercial CoverageThe company continues to add sales representatives across upper extremities, sports medicine, CMFT, robotics and foot and ankle. The plan to have at least 225 robotic sales specialists remains on track.
- Earlier Monogram Robotic System TimelineZimmer Biomet expects to file Monogram's 510(k) application with the FDA very soon and now anticipates launching a surgeon-guided, fully autonomous version in the first half of 2027, ahead of the original expectation of a 2028 launch. Management cited safety, efficiency, ease of use, reproducibility and accuracy as key platform claims, and reported zero patient safety reports in the clinical trial.
- Expanded Robotic Portfolio and THINK Surgical AgreementThe company extended its multiyear limited distribution and supply agreement with THINK Surgical for an exclusive cordless, wireless handheld TMINI robotic system compatible with Persona Knee. Management said the system requires minimal operating-room setup and complements the company's broader technology portfolio.
- Operational Efficiency InitiativesThe company is reshaping its U.S. manufacturing network by closing select underutilized sites, consolidating operations into facilities in Warsaw, Indiana, and reinvesting in advanced manufacturing processes. It is also consolidating international manufacturing into lower-cost geographies and implementing artificial intelligence capabilities with third-party experts.
- OpEx Improvement OpportunitiesManagement identified opportunities in process reengineering, shared services, demand planning, customer service, transactional work and AI-enabled productivity. A global capability center in India is operational, and the company expects sales force transformation investments to anniversary over time and provide operating leverage.
- Innovation, Ventures and Diversification StrategyZimmer Biomet launched a strategic venture platform to invest in early-stage technologies and plans to continue investing organically and inorganically in faster-growing markets. Its partnership with Deerfield Management and HSS includes a $300 million investment commitment over eight years, supporting an aspiration to increase weighted average market growth rates to 6% by the end of the decade.
- Disciplined M&A FrameworkManagement expects responsible M&A to be part of the 2027 strategy, targeting high-growth segments in reconstruction, higher-growth S.E.T. areas and adjacencies such as ambulatory surgery centers. The company generally targets acquisitions of up to $2 billion that are immediately revenue- and WAMGR-accretive, EPS-accretive by the end of year two and capable of generating double-digit ROIC by year five.
- Stable Market and ASC TrendsManagement maintained a global weighted average market growth rate of 4% to 4.25%, with a similar figure in the U.S., and did not see evidence of a structural market slowdown. Approximately 25% of the company's book of business goes to ambulatory surgery centers, where the company is seeing double-digit growth; large-joint ASC growth was described as upper single digit and, for hips, potentially double digit.
- Stable Implant Pricing IndicatorsManagement said implant cost as a percentage of the diagnosis-related group is at its lowest point in 22 years and that current pricing dynamics are steady. Third-quarter price erosion was 70 basis points, 10 basis points better than the second quarter and within the full-year guidance range of flat to 100 basis points.
