Q3 2026 Earnings Call Summary
The call was broadly positive, with management highlighting revenue growth, sequential improvements in collections, gross margins, operating expenses, losses, cash forecasts, AI adoption, and clinical pipeline progress. The main challenges were the 18% year-over-year decline in Precision Diagnostics revenue, the continued negative effects and unfinished work related to the billing transition, and ongoing operating losses and negative full-year operating margins.
Management reaffirmed full year revenue guidance of $330 million to $340 million for 2020, representing year over year growth of 2.3% to 5.4%, including Precision Diagnostics revenues of approximately $161 million to $165 million, anatomic pathology approximately $146 million to $150 million and biopharma services approximately $23 million to $25 million. Full year non GAAP gross margins are expected to be in the mid-30 percent range, non GAAP operating margins in the low negative 20s range, and therapeutic development investment approximately $21 million in 2026. Full year 2026 non GAAP loss is expected to be $1.65 to $1.75 per share versus prior guidance of a loss of $2.22 to $2.35 per share, assuming no further repurchases after the 4.1 million shares repurchased year to date. Assuming capital purchases of $13 million, therapeutic development investment of $21 million, and excluding any future stock repurchases or other expenditures outside the ordinary course which could include additional M&A, ending cash is anticipated to be approximately $630 million versus a prior forecast of $610 million.
- Therapeutic Pipeline ProgressFID-007's Phase 2 study is majority complete, and Fulgent held its end-of-Phase 2 meeting with the FDA in August. The Phase 3 study is in preparation, with management viewing the interim Phase 3 registration trial as an important step toward potential commercialization for recurrent or metastatic head and neck squamous cell carcinoma. The company is also continuing discussions with potential partners.
- FID-022 Phase 1 AdvancementFID-022 continued to progress through Phase 1 dose escalation, with the maximum tolerated dose expected to be determined in the first half of next year. The nano-encapsulated SN-38 candidate is being developed for solid tumors, including potentially colon, pancreatic, and bile duct cancers.
- Laboratory Services Revenue GrowthThird-quarter revenue was $90.4 million, up 8% year over year and 6% sequentially. The continued integration of Bako Diagnostics and StrataDx contributed to overall performance.
- Anatomic Pathology and Biopharma Services GrowthAnatomic pathology revenue was $40.9 million, increasing 57% year over year and 9% sequentially. Biopharma services revenue was $7.7 million, increasing 5% year over year and 19% sequentially.
- Improvement in Revenue Cycle ManagementFulgent made significant progress addressing billing and revenue cycle management delays. Required system customizations improved collection rates, contributing to sequential improvements in total revenue and gross margins. Management said it has a clear line of sight into what is needed to return to historical collection rates, although additional fine-tuning remains.
- PAMA Exposure Expected to Be MinimalBased on current information, management expects the impact of potential PAMA and clinical lab fee schedule changes to be minimal. The company's primary CPT codes have limited exposure to the reimbursement reductions currently contemplated, and PAMA is not currently viewed as a material headwind to the revenue or earnings outlook.
- AI Platform Scaling and ReliabilityThe AI platform analyzed approximately 255,000 slide images in the third quarter, a 23% increase from the second quarter. Failed analyses declined to 0.01%. Four AI modules are in production, and five additional AI applications are planned for deployment in the fourth quarter.
- Prostate Cancer AI Module PerformanceIn internal testing across 262 cases, the prostate cancer application identified 768 of 770 cancer-containing parts. The module is undergoing pathologist evaluation.
- New AI Architecture ImprovementsFulgent transitioned its production environment to a new AI analysis platform designed to improve speed, reliability, and scalability. The new architecture meaningfully reduced processing times across several high-volume applications and is intended to support additional AI modules moving into production.
- Sequential Gross Margin ImprovementGAAP gross margin was 35.3% and non-GAAP gross margin was 36.3% in the third quarter. The sequential improvement reflected fixed-cost leverage over a higher revenue base, driven by better collection rates.
- Lower Operating Expenses and Improved MarginsGAAP operating expenses declined to $54.5 million from $61.8 million in the second quarter, while non-GAAP operating expenses declined to $44.8 million from $49.2 million. GAAP operating margin improved to negative 24.9% from negative 42.3%, and non-GAAP operating margin improved to negative 13.2% from negative 26.2%.
- Reduced Quarterly LossesGAAP loss decreased to $13.2 million from $29.5 million in the prior quarter, or $0.48 per diluted share. The non-GAAP loss was approximately $2.4 million, or $0.09 per share, and adjusted EBITDA loss improved to approximately $7.5 million from $17.1 million.
- Strong Cash Position and Tax RefundFulgent ended the quarter with approximately $652.4 million in cash, cash equivalents, restricted cash, and marketable securities. Cash increased by $100.9 million sequentially, primarily due to receipt of a $112.3 million federal tax refund and related interest, partially offset by $11.4 million used in operations and capital expenditures.
- Share Repurchase ActivityThe company repurchased just over 15,000 shares during the third quarter. Since the program began in March 2022, more than 7.5 million shares have been repurchased, with approximately $75.6 million remaining available for future purchases.
- Revenue Guidance ReaffirmedFull-year revenue guidance was reaffirmed at $330 million to $340 million, representing year-over-year growth of 2.3% to 5.4%. Segment expectations are approximately $161 million to $165 million for Precision Diagnostics, $146 million to $150 million for anatomic pathology, and $23 million to $25 million for biopharma services.
- Improved Full-Year Loss GuidanceFull-year 2026 non-GAAP loss guidance improved to $1.65 to $1.75 per share from the prior guidance of a loss of $2.22 to $2.35 per share. Management attributed the improvement to disciplined spending and lower operating expenses.
- Increased Year-End Cash ForecastThe company increased its expected year-end cash, cash equivalents, restricted cash, and marketable securities balance to approximately $630 million from a prior forecast of $610 million. The increase was attributed to the improved operating outlook and lower therapeutic development spending.
- Continued Strategic InvestmentFulgent expects approximately $21 million of therapeutic development investment in 2026 to advance FID-022 and FID-007. The company is also prioritizing investment in laboratory testing capabilities, clinical studies, AI and digital solutions, laboratory infrastructure, and the integration of Bako Diagnostics and StrataDx.
