Q3 2026 Earnings Call Summary
The call was strongly positive. Dexcom reported 11% worldwide revenue growth, 14% U.S. growth, continued market-share gains, near-record new patient starts, significant gross-margin and EBITDA-margin expansion, strong cash generation, raised operating and EBITDA margin guidance, and substantial progress in clinical evidence, product launches, manufacturing, and future access expansion. The main challenges were slower international growth, near-term Ireland ramp effects, incremental foreign-exchange headwinds, higher operating expenses, and the loss of preferred access under a PBM representing less than 4% of U.S. revenue. Highlights significantly outweighed the lowlights.
Dexcom updated full year revenue guidance to be in the range of $5.20 billion to $5.25 billion, representing growth of 12% to 13% for the year, with midpoint fourth quarter growth of 10%; it maintained full year non-GAAP gross profit margin guidance at approximately 64% and increased non-GAAP operating profit margin guidance to approximately 24% and adjusted EBITDA margin guidance to approximately 32%. Dexcom expects to convert nearly 50% of its U.S. customer base to G7 15 Day by year-end, expects CMS to announce a type 2 non-insulin coverage decision this year with coverage taking effect middle of next year, and is still targeting GA for either launching at the end of next year or early '28, depending on regulatory review time lines.
- Third-Quarter Revenue GrowthWorldwide third-quarter revenue was $1.34 billion versus $1.21 billion in the third quarter of 2025, representing 11% growth on both a reported and organic basis.
- Strong U.S. Performance and Share GainsU.S. revenue increased 14% to $970 million from $852 million, with Dexcom capturing share across all customer segments. The largest share increase came from customers with type 2 diabetes, including those not on insulin.
- Near-Record New Patient StartsGlobal new patient starts were near record levels in the third quarter, and new customer starts were up year over year. Management said the company is tracking toward a record new patient year.
- Stable Category Growth and Active-Base ExpansionManagement reported stable low double-digit category revenue growth, active-base growth in the upper teens, and active-base growth above revenue growth. The U.S. market remained stable, with the two largest CGM players combined still generating low double-digit revenue growth.
- CONNECT Clinical EvidenceIn the six-month CONNECT randomized controlled trial, the Dexcom CGM group achieved a 1.6% A1c improvement, representing a 0.9% A1c difference versus the control group. Management described these as the strongest A1c results generated across its historic randomized controlled trials.
- New England Journal of Medicine PublicationCONNECT was published in the New England Journal of Medicine, providing independent validation and a prominent platform among clinicians, advocacy organizations, and reimbursement bodies. Management noted that this was the first publication of stand-alone CGM clinical data in the journal in nearly two decades.
- Progress Toward Broader Medicare CoverageDexcom continues to expect CMS to announce a type 2 non-insulin coverage decision this year, with coverage currently expected to take effect in the middle of next year. Management said the case for reimbursement is strengthening and provided the CONNECT publication update directly to Medicare medical directors.
- Broader Metabolic-Health EvidenceManagement cited significant improvements across multiple studies in 10-year cardiovascular disease risk, blood pressure, cholesterol, triglycerides, and kidney-disease progression, supporting broader CGM benefits across metabolic health.
- Redesigned Stelo AppDexcom launched a fully redesigned Stelo app during the quarter with a more intuitive and personalized experience, enhanced glucose insights, simplified meal logging, and more detailed nutritional information. The company said the redesign establishes a software foundation for future product capabilities and greater personalization.
- G7 15 Day AdoptionBroad pump connectivity for the G7 15 Day system became available during the summer, and customer adoption steadily built throughout the quarter. Dexcom remains on track to convert nearly 50% of its U.S. customer base to G7 15 Day by year-end.
- G7 15 Day Product PerformancePhysician and customer feedback on the G7 15 Day system remained positive, particularly regarding extended wear time, sensor performance, and the updated patch design. The updated adhesive has produced a 4- to 5-point improvement in 15 Day survivability.
- International G7 15 Day ExpansionHealth Canada cleared G7 15 Day earlier this year, and Dexcom plans to launch the product in Canada beginning in November. The company also received clearance in Japan during the third quarter and is targeting a launch there in early 2027.
- Manufacturing and Supply-Network ProgressDexcom delivered further manufacturing efficiencies, improved freight costs, and strong product performance in the field. Its Ireland facility initiated commercial production on schedule, expanding manufacturing capacity and strengthening the global supply network across the U.S., Europe, and Asia Pacific.
- Digital Customer-Support ExpansionDexcom is expanding its digital customer-support ecosystem, including My Dexcom Account, into select international markets. The company said these tools simplify support, improve efficiency, and save time for customers; the platform has rolled out in Europe and is expected to roll out in APAC shortly.
- Gross-Margin ExpansionThird-quarter gross profit was $875.3 million, or 65.1% of revenue, compared with 61.3% in the third quarter of 2025. Gross margin increased approximately 380 basis points, driven by manufacturing efficiencies, progress in freight and quality management, and the ramp-up of G7 15 Day in the U.S.
- Operating and EBITDA Margin ExpansionOperating income was $339.5 million, or 25.3% of revenue, versus $272.9 million, or 22.6% of revenue, a year earlier. Adjusted EBITDA was $435.2 million, or 32.4% of revenue, versus $368.4 million, or 30.5% of revenue, in the prior-year quarter.
- Net Income and Cash PositionThird-quarter net income was $275 million, or $0.73 per share. Dexcom ended the quarter with approximately $1.8 billion of cash and cash equivalents.
- Strong Free Cash Flow and Share RepurchasesDexcom generated more than $900 million of free cash flow through the first three quarters of the year and completed its Investor Day goal of repurchasing $1 billion of stock in 2026. The company authorized another $1 billion share repurchase program in line with its commitment to deploy at least 50% of free cash flow generated during 2027.
- International Distribution AcquisitionDexcom signed an agreement to acquire a business from an international distribution partner, allowing it to go direct across high-potential European markets. Management described the transaction as a growth lever and the company's largest remaining distributor market in Europe; closing is expected next year.
- Raised Full-Year GuidanceRevenue guidance was updated to $5.20 billion to $5.25 billion, representing 12% to 13% growth for the year. Non-GAAP operating profit margin guidance increased to approximately 24%, and adjusted EBITDA margin guidance increased to approximately 32%, while gross-margin guidance remained approximately 64%.
- Stable Pricing EnvironmentManagement said it has not seen significant changes to its pricing environment, including outside the U.S., and stated that pricing and reimbursement have remained stable in the markets where Dexcom operates.
- Stable Retention and UtilizationRetention and utilization remained stable across a growing customer base, including a larger proportion of type 2 non-insulin users. Management attributed this to work on customer success, product experience, and support initiatives.
- Smart Basal ProgressThe Smart Basal pilot produced positive patient and prescriber satisfaction and outcomes. Most patients reached their optimum basal dose in about three weeks with no hypoglycemia, and Dexcom is expanding compatibility beyond insulin glargine to include degludec and other higher-concentration insulins.
- G8 Development ProgressDexcom reported strong clinical-study results for G8 accuracy and performance. The platform has a new electronics sensor in a 50% smaller volume with more capability than G7, and the company remains on track for a launch at the end of next year or in early 2028, subject to regulatory review timelines.
- Operating Structure and Future InvestmentJereme Sylvain was promoted to Chief Operating Officer in addition to his Chief Financial Officer responsibilities, bringing operations, quality, and customer success closer to financial and strategic priorities. Management also plans to continue investing in G8 clinical work, Dexcom Direct, international expansion, and other growth initiatives while pursuing operating leverage.
