Q1 2027 Earnings Call Audio
Q1 2027 Earnings Slide Deck
Q1 2027 Earnings Call Summary
The call presented a credible long-term opportunity supported by new clinical standards, expanded FDA indications, broad reimbursement coverage, higher renewal pricing, ARR growth, rising patient testing and a strengthened leadership team. However, these positives were offset by sales below expectations, declining TCV, no Heart Health sales, body composition sales shortfalls, sales-team execution gaps and a potentially delayed path to cash-flow breakeven. The overall tone was constructive but balanced, with management expressing confidence in the strategy while acknowledging significant near-term execution challenges.
For Q2, the company said its pipeline “remains strong and continues to grow, and it's clearer and cleaner than ever,” but there is a “ramp to meaningful countrywide productivity and to the larger system-wide deals,” with the team down 4 sales reps at the start of the quarter and hiring underway; the January 2028 compliance date for the NAPBC standard is the priority commercial target, with the company currently only 25% penetrated in those sites. Heart Health investment will remain “deliberately paced” until the segment demonstrates the momentum to warrant investment at scale, while the path to cash flow breakeven “may be impacted” and the company is “actively evaluating a range of initiatives to mitigate that.” Contracts in place at 30 September '26 are expected to generate ARR of $14.9 million for the 12 months to 30 September '27, while the cost-reduction actions target approximately $5 million on a full year basis.
- Regulatory-Cleared Platform and Scalable InfrastructureThe company has an FDA-cleared, TGA-cleared and CE-marked digital health platform, with 662 devices in leading U.S. health care systems and 27 IDN partnerships representing more than 1,400 hospital opportunities. Management said the infrastructure can be leveraged across cancer, Heart Health and body composition without rebuilding from scratch.
- National Reimbursement Coverage Remains BroadThere was no change in reimbursement levels during the quarter. National coverage remained above 95%, representing 331 million covered lives, which management said supports new customer acquisition and reengagement with existing accounts.
- New NAPBC Lymphedema Surveillance StandardThe National Accreditation Program for Breast Centers released its 2027 standards, including required Standard 5.8 for lymphedema surveillance programs covering breast cancer patients at risk across 540 accredited centers. Management said SOZO can support three standards with one program and identified a January 2028 compliance date as a priority commercial opportunity.
- Opportunity to Expand NAPBC PenetrationThe company said it is currently only 25% penetrated in NAPBC sites, leaving an identified opportunity to expand adoption as centers prepare for the new required surveillance standard.
- Sarcopenia Risk Assessment FDA Clearance Broadens Clinical ReachThe recent FDA clearance for sarcopenia risk assessment expands SOZO's relevance before, during and after cancer treatment and opens access to stakeholders beyond breast surgeons, including medical oncology, nurse navigation and cancer service-line directors.
- Strengthened Commercial and Clinical LeadershipThe company said it now has a complete commercial leadership team, including Erik Johnson as Head of Sales and Nicole Stout as Chief Clinical Innovation Officer effective November 1. Nicole brings almost 30 years of experience in breast cancer, cancer survivorship and lymphedema, while Erik brings more than 15 years of medtech sales leadership experience, including 12 years at Hologic.
- Higher Renewal PricingAverage pricing increased 18% for contracts renewed during the quarter. Management also said average pricing attached to contract renewals continues to grow.
- ARR Increased 12% Year Over YearContracts in place at September 30, 2026 were expected to generate ARR of $14.9 million for the 12 months to September 30, 2027. In constant currency terms, ARR was $16.1 million, up 12% compared with Q1 FY2026.
- Patient Testing and Installed Base Continued to GrowThe installed base reached close to 1,300 units, including 662 units placed in the U.S. Patient testing increased 8% from the prior quarter and had a three-year compound growth rate of 17%.
- Operating Cash Outflow DeclinedTotal operating cash outflow was $2.5 million, in line with expectations and down $2.4 million from the prior quarter. Product-related costs declined $800,000, primarily due to the timing of inventory purchases, while staff costs declined $1.2 million compared with Q4.
- Cost-Base Reduction and Cash PositionManagement said Q1 net cash outflow reflected actions implemented to reduce the operating cost base by approximately $5 million on a full-year basis. Cash at September 30 was $11.6 million, equivalent to 4.7 quarters of operating cash flow.
- Heart Health Clinical Evidence ProgramAlthough there were no Heart Health sales in the quarter, the company is working with a large health system whose 44 heart failure clinics are actively enrolling patients in two studies: one involving frailty in heart patients and another comparing SOZO with a market-leading implantable pulmonary artery pressure monitoring system. The company is also pursuing an FDA application for additional cardiovascular conditions and bench-testing collaborations with leading device companies.
- Body Composition Partnerships and Market ActivityManagement identified opportunities across primary care, sports medicine and research, wellness and longevity, and medically managed weight loss. The company is partnering with Dr. Jessica Shepherd, who will deliver an education session with ImpediMed at the American College of Lifestyle Medicine Conference, and plans to participate in Obesity Medicine and Wellness and Longevity conferences.
- Pipeline and Strategic PartnershipsManagement said the cancer survivorship pipeline remains strong, continues to grow, and is clearer and cleaner than before. The company is also solidifying strategic and key opinion leader partnerships in weight management and body composition and pursuing larger strategic players to drive scale more efficiently.
