Q3 2026 Earnings Call Summary
The call was strongly positive, led by record total and Tyvaso revenue, improving referral and demand trends, favorable intellectual property and settlement developments, and confidence in a broad pipeline and upcoming launches. The main negatives were execution and regulatory timing issues, including the delayed soft mist inhaler filing, the ralinepag API supplier’s OAI status, uncertainty around the triple oral pill filing path, and moderate near-term patient switching expectations.
Management expects near-term transitions from YUTREPIA to Tyvaso to be moderate, with a PH-ILD referral inflection expected over the next couple weeks, and into December, and then obviously the first part of the year, followed by a new baseline in referrals over the next few weeks; it expects to submit the soft mist inhaler NDA in 2027 and not at the end of this year, while oral ralinepag has a PDUFA date of June 24 of next year. Management also expects nebulized Tyvaso in IPF and ralinepag in PAH to become multi-billion dollar growth drivers, expects to enter 2027 with substantially more momentum than entering 2026, and reported $833 million of third-quarter total revenue, $504 million of Tyvaso revenue, over 1,000 referrals in October, and that one in three patients starting TYVASO DPI since the introduction of higher-strength cartridges achieved higher doses, twice as what it was beforehand.
- Record Third-Quarter RevenueUnited Therapeutics delivered record total revenue of $833 million in the third quarter of 2026, which management described as the company’s best quarter ever.
- Record Tyvaso Revenue and Renewed MomentumTyvaso revenue reached a record $504 million, supported by strong sequential growth and underlying demand. Management said the business has regained meaningful momentum and expects to enter 2027 with substantially more momentum than it had entering 2026.
- Tyvaso Demand Drivers and Commercial ExecutionTyvaso performance was driven by starts, referrals, patient demand, increased dosing, physician engagement, and strong execution by the commercial and medical teams. Management said there were no meaningful inventory or seasonal dynamics affecting the quarter.
- Higher-Dose Tyvaso DPI AdoptionThe 80-microgram cartridge and higher-dose kits helped address misinformation about the ability to reach higher doses with Tyvaso DPI. Among patients who started Tyvaso DPI since those products were introduced, approximately one in three achieved the higher doses, twice the prior level.
- Sales Force Expansion Showing Early BenefitsEarlier investments in resources and messaging are expanding reach, deepening physician engagement, increasing disease awareness, and driving penetration across PAH and PH-ILD. Management said the sales force expansion contributed to consistent month-to-month performance and an inflection in referrals and starts beginning around mid-September.
- Record Referral ActivityUnited Therapeutics received more than 1,000 referrals for a company product in October, the most referrals received in a single month.
- Favorable Intellectual Property SettlementA court decision upheld United Therapeutics’ intellectual property, and the related settlement removed PH-ILD from the YUTREPIA label, prevented promotion and marketing into that indication, and prohibited Liquidia from encouraging off-label usage. Management said the terms were highly favorable and provide important clarity around the long-term durability of the base business.
- Large PH-ILD Opportunity RemainsWith no direct branded competition on the horizon, management said Tyvaso can remain focused on growing the large, under-penetrated PH-ILD market through greater physician awareness, identification of eligible patients, and new prescriptions.
- Broad Development Pipeline and Market OpportunityUnited Therapeutics has 14 programs in development across nebulized and dry powder therapies, soft mist inhalers, once-daily medicines, and combination products. Across IPF, PPF, and PH-COPD, the company is pursuing patient populations totaling approximately 1 million people in the United States.
- Long-Dated Competitive ProtectionsThe portfolio includes fibrosis-related patents, device protections, and composition-of-matter protection for ralinepag, with many protections extending into the 2040s. Management also cited its decades of expertise, physician engagement, specialized commercialization capabilities, manufacturing investments, and experience bringing complex drug-device combinations to market.
- Potential Future Growth DriversManagement identified nebulized Tyvaso in IPF and ralinepag in PAH as two important potential launches. Once approved, both are expected to address significant unmet need and have the potential to become multi-billion-dollar growth drivers.
- Progress on Manufactured Organ AlternativesThe company continues to advance its manufactured organ alternatives programs and build the infrastructure needed to address transplantation needs, which management described as a parallel pillar of its long-term vision.
- PH-COPD Program Design and IND ClearanceThe PH-COPD Phase II trial has IND clearance. Management said the study uses a more robust design and an enriched patient population developed with steering committees, with inclusion and exclusion criteria intended to improve the chances of success and decrease program risk.
- Ralinepag Royalty Buyout Reflects ConfidenceUnited Therapeutics bought down Arena’s economics for ralinepag, which management said demonstrates confidence in the revenue opportunity for oral ralinepag and in the dry powder inhalation formulations being developed for multiple pulmonary indications. The transaction was described as an opportunistic use of capital.
- Rali DPI Development PlansThe company is conducting IND-enabling studies and planning a Phase I pharmacokinetic and safety study in healthy volunteers for Rali DPI. The initial plan is to pursue PH-ILD and, either very soon thereafter or essentially at the same time, seek approval for IPF and PPF.
