Q3 2026 Earnings Call Summary
The call was strongly positive, with solid third-quarter revenue growth, raised 2026 guidance, strong performance from KEYTRUDA, WELIREG, WINREVAIR, Gardasil, and animal health, and multiple favorable clinical and regulatory milestones. Management expressed increased confidence in the pipeline and long-term growth beyond the KEYTRUDA loss-of-exclusivity period. Offsetting concerns included 2027 generic and loss-of-exclusivity headwinds, moderating KEYTRUDA growth, lower adult vaccination utilization, modestly lower gross margin, higher operating expenses, the QLEX European injunctions, and withdrawal of the ifinatamab deruxtecan application. Highlights significantly outweighed the lowlights.
For 2026, Merck expects revenue between $66.8 billion and $67.3 billion, representing growth of 3% to 4% including a positive impact from foreign exchange of approximately 1 percentage point using mid October rates; gross margin approximately 81%; operating expenses between $42.4 billion and $42.9 billion; other expense approximately $1.4 billion; a full year tax rate between 32-33%; approximately 2.48 billion shares outstanding; and EPS of $2.77 to $2.82 including a positive impact from foreign exchange of approximately $0.15 using mid October rates, with the Ceybrion charge included. The company expects approximately $3 billion in share repurchases this year, moderate top line growth in 2027, more than $2 billion of headwinds from generic competition and LOE, a $250 million CDC buy unwinding in 2027, underlying operating expenses to increase at a mid to high single digit rate, and a shallow dip in revenues in the initial period post the price setting through IRA and the LOE, followed by a quick return to growth; it also expects 30% to 40% QLEX use in the United States by the end of 2027, large coverage of access in the commercial space in the 2027 time frame, and Medicare coverage in the 2028 time frame.
- Third-Quarter Revenue GrowthThird-quarter revenue was $18.1 billion, an increase of 5%, or 4% excluding foreign exchange, driven by increasing contributions from new products and continued strength in oncology and animal health.
- KEYTRUDA Franchise Continued to GrowSales of the KEYTRUDA family, including KEYTRUDA and KEYTRUDA QLEX, increased 6% to $8.6 billion on an ex-exchange basis, driven by uptake in earlier-stage cancers, continued demand in metastatic indications, utilization in cervical and breast cancers, increased KEYTRUDA plus PADCEV use in urothelial cancer, and uptake in earlier-stage head and neck cancer.
- KEYTRUDA QLEX Adoption IncreasedKEYTRUDA QLEX sales were $830 million, with adoption continuing to increase. In the United States, the company reported a positive trajectory following the permanent J code received on April 1 and retained an objective of reaching 30% to 40% utilization by the end of 2027.
- WELIREG Delivered Strong GrowthWELIREG sales increased 58% to $370 million, driven by increased use in previously treated patients and as adjuvant treatment for certain renal cell carcinoma patients in the United States, as well as ongoing international launches.
- Gardasil Sales IncreasedGardasil sales increased 14% to $2.0 billion. U.S. sales grew 18%, benefiting from a significant CDC purchase and price, while international sales increased 6%.
- WINREVAIR Momentum ContinuedWINREVAIR sales increased 94% to $694 million, reflecting strong demand from adults with pulmonary arterial hypertension and a benefit from channel inventory. Nearly 1.6 thousand new U.S. patients received a prescription, and approximately 30% were not on therapies that included a prostacyclin.
- OHTUVAYRE Demand Remained PositiveOHTUVAYRE sales were $184 million, reflecting continued demand from adults with COPD, partially offset by a small headwind from the timing of specialty pharmacy purchases.
- Animal Health Business GrewThe animal health business grew 3%. Companion animal sales increased 6%, driven by new product launches, while livestock sales grew 2% due to higher demand for poultry products.
- Positive Early LIPFENDRA Prescription TrendsLIPFENDRA's early launch showed positive prescription trends and physician feedback. In the third quarter, the company recorded more than 11 thousand prescriptions from over 6.5 thousand prescribers, including cardiologists and general practitioners. Most prescriptions came from patients using other background lipid-lowering therapies rather than switching from injectable PCSK9 therapies.
- Full-Year Revenue and EPS Guidance RaisedThe company narrowed its 2026 guidance range and raised the midpoint. Revenue is now expected to be between $66.8 billion and $67.3 billion, representing growth of 3% to 4% including an approximately 1-percentage-point positive foreign-exchange impact. EPS is expected to be $2.77 to $2.82, including an approximately $0.15 positive foreign-exchange impact and the Ceybrion charge.
- Third-Quarter Earnings PerformanceNon-GAAP earnings per share were $2.49, including a $0.12-per-share charge related to the Ceybrion license agreement. The tax rate was 10.9%, reflecting a benefit from certain discrete items.
- Strong Oncology Clinical ProgressAt ESMO, the company presented the first positive Phase III results from the GloboTROFuse program for sac-TMT in advanced or recurrent endometrial cancer. The GloboTROFuse-O5 study demonstrated a near 7-month improvement in median overall survival versus chemotherapy alone.
- Sac-TMT Development Expanded Across Tumor TypesPartner Kelun's data showed a meaningful progression-free-survival improvement for sac-TMT plus KEYTRUDA over KEYTRUDA plus chemotherapy in first-line PD-L1-negative nonsquamous non-small-cell lung cancer. Merck has five ongoing Phase III studies of sac-TMT in various non-small-cell lung cancer settings and anticipates Phase III studies with MK-2020 in the first half of 2027.
- Cancer Vaccine Data Supported Further DevelopmentPhase III data for entismodigene, an individualized neoantigen cancer vaccine developed with Moderna, plus KEYTRUDA showed clinically meaningful improvements in recurrence-free survival and distant-metastases-free survival versus standard-of-care KEYTRUDA in completely resected stage IIb-IV melanoma. Merck is actively engaged with the FDA on a regulatory submission.
- Additional Oncology and Regulatory MilestonesSubcutaneous pembrolizumab with vorhyaluronidase alfa was approved in Japan and China. Europe approved KEYTRUDA plus PADCEV as perioperative treatment for resectable muscle-invasive bladder cancer, and the U.S. FDA approved WELIREG plus LENVIMA for previously treated renal cell carcinoma.
- WINREVAIR Label Expansion and New Phase III StudyThe FDA approved a WINREVAIR label update incorporating Phase III Hyperion results and supporting earlier use in adults with pulmonary arterial hypertension. Merck plans to initiate the Phase III CORAAL study in combined post- and precapillary pulmonary hypertension associated with heart failure with preserved ejection fraction, using a 0.3 milligram-per-kilogram dose and time to clinical worsening as the endpoint.
- Positive Immunology and Ophthalmology DataPhase IIb results for tultokibart in hidradenitis suppurativa showed promising clinical responses and an encouraging safety profile, making it the first anti-TL1A antibody to demonstrate positive clinical data in a dermatology indication. The company plans to advance it into Phase III. In ophthalmology, the Phase IIb/III Brunello study of remygrimeg achieved its primary endpoint, with both monthly doses demonstrating noninferior best-corrected visual acuity versus monthly 0.5 milligram ranibizumab at one year.
- Broad Pipeline Milestones AheadOver the next 12 months, Merck expects milestones including tultokibart Phase III ATLAS UC data, the Barolo study readout for remygrimeg, initiation of the CORAAL study, potential filings for islatravir plus lenacapavir, data from the ANCHOR influenza-prevention study, data from the expressive HIV PrEP program, potential sac-TMT approval in endometrial cancer, additional Phase III oncology readouts, and a potential entismodigene regulatory filing.
- KRAS Development Strategy AdvancedMerck described MK-1084 krasirag as potent, selective, and suitable for combination with KEYTRUDA in lung cancer and cetuximab in colorectal cancer. The company is pursuing a chemotherapy-free first-line lung cancer strategy using KEYTRUDA plus the KRAS G12C inhibitor and expects important clinical data for its KRAS G12D program in the next year or so.
- Long-Term Revenue Opportunity IncreasedManagement said recent positive clinical results increased confidence in the previously discussed $70 billion mid-2030s revenue opportunity and indicated that it expects to raise that number. The opportunity did not originally include the Prometheus acquisition, included little for the PD-1/VEGF program, and may benefit from broader sac-TMT indications and a stronger-than-expected entismodigene product profile.
- Capital Allocation and Financial FlexibilityMerck reiterated its commitment to investing in value-creating opportunities, increasing the dividend over time, and pursuing science-driven business development transactions. The company remains on track for approximately $3 billion in share repurchases in 2026 and retains financial flexibility within a strong investment-grade credit rating.
