Q3 2026 Earnings Call Summary
The call was broadly positive. Third-quarter organic revenue growth was 6.7%, both major product lines grew organically, AirSeal, direct Buffalo Filter, and BioBrace performed well, and adjusted EPS exceeded expectations with higher full-year EPS guidance. The main negatives were declining reported revenue and adjusted EPS, lower free cash flow, weaker U.S. orthopedic and OEM smoke-evacuation sales, higher interest expense, tariff exposure, and a narrowed high end of revenue guidance. Overall, the highlights significantly outweighed the lowlights.
For full year 2026, ConMed expects at least 5% organic constant currency revenue growth, with net sales growth of 5% to 5.5%, GI revenue of approximately $20 million, and changes in foreign currency exchange rates representing a tailwind to GAAP results of approximately $9 million; non GAAP adjusted diluted earnings per share are expected in the range of $4.54 to $4.61, adjusted gross margin of approximately 57.5%, adjusted interest expense of approximately $33.7 million, adjusted effective tax rate of approximately 24.5%, free cash flow of approximately $115 million, weighted average shares outstanding of 30.2 million, and $62 million allocated to share repurchase. Fourth-quarter guidance is GAAP net sales of between $364 million and $370 million, organic constant currency growth in the range of 5.1% to 6.8% excluding expected GI revenue of approximately $3.7 million and an FX tailwind of approximately $650 thousand, and adjusted EPS of between $1.23 and $1.30; the company also expects a $0.35 tariff impact ignoring the tariff refunds and 50 to 100 basis points of gross margin expansion with and without the refund.
- Strong Organic Revenue GrowthThird-quarter net revenue was $335.6 million. Organic constant currency revenue increased 6.7% year over year, consistent with expectations, with balanced contributions from general surgery and orthopedic surgery.
- Adjusted EPS Exceeded ExpectationsThird-quarter adjusted diluted EPS was $1.04, exceeding the high end of the company's expectations by $0.01 and the low end by $0.06.
- General Surgery Growth Led by International MarketsGeneral surgery sales increased 6.4% on an organic constant currency basis, driven by 15.5% international growth, including strong performance in EMEA and APAC, and 1.9% domestic growth.
- AirSeal Growth AcceleratedAirSeal sales accelerated in the third quarter and grew within the targeted high single digit to low double digit range. Growth was driven by stable disposable sales and strong capital-equipment growth, particularly outside the United States.
- AirSeal Commercial and Clinical ExpansionConMed received an expanded U.S. clinical indication for AirSeal's robotic solution with Intuitive's 8 millimeter hex cannulas, supporting use with the da Vinci 5 platform. The company is also expanding procedure-specific training, clinician relationships, and clinical and economic evidence in robotic and laparoscopic surgery.
- AirSeal Has Significant Laparoscopic OpportunityAirSeal is estimated to be used in only 6% to 7% of more than 3 million annual laparoscopic procedures in the United States. Management is targeting selected procedure categories and surgeon groups where it believes AirSeal can provide the greatest clinical and economic impact.
- Direct Buffalo Filter Growth and Legislative TailwindsDirect smoke evacuation sales grew within the high single digit to low double digit range. Michigan enacted a law requiring facilities to implement smoke evacuation policies by July 2027, and Pennsylvania enacted a law requiring hospitals and ambulatory surgery centers to use smoke evacuation devices during procedures generating hazardous smoke. There are now 23 states with smoke-free operating-room laws covering approximately 60% of the U.S. population, while more than 10 additional states are considering similar legislation.
- Orthopedic Surgery Delivered Broad International GrowthOrthopedic surgery organic constant currency sales increased 7.2%, driven by 12.7% international growth across major regions.
- BioBrace Continued to Drive Orthopedic GrowthBioBrace was a key driver of orthopedic sales, particularly internationally. The company cited a clinically demonstrated 94% healing rate when BioBrace is used to augment rotator cuff repair in high-risk patients, and the American Academy of Orthopedic Surgeons' clinical practice guidelines strongly recommend bioinductive implants for these procedures.
- BioBrace Global ExpansionConMed secured regulatory clearance for BioBrace in Brazil during the third quarter. BioBrace is approved for rotator cuff applications in Canada, certain Eastern European and Middle Eastern markets, Korea, China, India, and Brazil, with studies ongoing for additional international indications.
- Improved Gross and Operating MarginsThird-quarter adjusted gross margin increased 40 basis points to 56.4%, driven primarily by favorable product mix. Adjusted operating margin increased 20 basis points to 15.4%, while adjusted SG&A decreased 20 basis points and adjusted R&D decreased 1%.
- Raised Adjusted EPS GuidanceFull-year 2026 adjusted diluted EPS guidance increased to $4.54 to $4.61 from the prior range of $4.48 to $4.60. The company expects fourth-quarter adjusted EPS of $1.23 to $1.30.
- Maintained Minimum Organic Growth OutlookManagement reaffirmed confidence in achieving at least 5% organic constant currency revenue growth in 2026. Full-year organic constant currency growth guidance is now 5% to 5.5%, and fourth-quarter organic constant currency growth is expected to be 5.1% to 6.8%, excluding expected GI revenue of approximately $3.7 million.
- Expected Full-Year Gross Margin ExpansionThe company expects full-year 2026 adjusted gross margin of approximately 57.5% and stated that it expects to achieve 50 to 100 basis points of gross-margin expansion for the full year.
- Balance Sheet and Share RepurchasesAs of September 30, 2026, cash was $48 million, total debt obligations were $815.9 million, available borrowing capacity was $468.5 million, and leverage was 2.7 times. ConMed repurchased approximately 1.2 million shares for $49.9 million during the first nine months and expects to allocate $62 million to share repurchases for the full year.
- Strategic Focus on Three Growth MarketsFollowing the strategic exits from GI products, management said the company is increasingly focused on minimally invasive surgery, smoke evacuation, and orthopedic soft-tissue repair, with AirSeal, Buffalo Filter, and BioBrace as its three key growth platforms.
- Future Cash Flow and M&A PrioritiesManagement highlighted ConMed's historical free-cash-flow conversion of greater than 90% in 7 of the last 10 years and identified opportunities in inventory, interest expense, working capital, and nonrecurring cash spending. It also stated that the company has an appetite for revenue-growth-accretive M&A opportunities and will provide additional capital-allocation specifics on the fourth-quarter call.
